How tor refer your patient

There are four procedures that you must follow to refer a patient to a Sonorex Therapy Clinic.

1. Evaluate the patients muscular pain as you would normally do.

2. Make sure the patient meets criteria and has no contraindications for Sonocur® Extracorporeal Shockwave Therapy Treatment.

3. Complete the "Sonorex Extracorporeal Shockwave Therapy Treatment Request Form" for your patient to take to the Sonorex Therapy Center.

4. Call 1-877-766-6287 to schedule an appointment at one of our Sonorex

The Sonocur® Orthopedic Extracorporeal Shockwave system is available in Canada and other countries where regulatory approval has been obtained. The Sonocur® Basic is FDA approved in the United States for the treatment of chronic lateral epicondylitis (tennis elbow).
